Student finance forms and guides 2008/2009

As well as applying online, you can apply for student finance by completing a paper form. Students and their parents, husbands, wives and partners can download forms and guidance notes for academic year 2008/2009 below.

Proof of identity

New students applying for support in 2008/2009 should submit their passport as proof of identity. If you apply using the online service, you can submit these details online without having to send in your passport through the post.

If you don’t hold a passport you can submit your birth or adoption certificate, but you must also complete and return the birth/adoption certificate declaration form.

The form contains two separate declarations. The 'applicant's declaration of identity' must be signed by you (the student) to confirm your identity. The 'identity confirmation form' is to verify your identity and must be completed by a person of good standing in the community who has known you for at least two years.

If you have already applied for support and now wish to amend either the amount you requested through the Student Loan for Maintenance or Student Loan for Tuition Fees, you will need to submit one of the forms below.
